Why this one is dangerous

The employee has treated a generated answer as an instruction. There is no record of what was asked, no record of what came back, and no named person who checked it. When the decision is later questioned, the organisation cannot show how it was reached.

Regulators and courts do not accept a model as a decision maker. The accountability sits with the company either way.

How to combat it

  • Name a human owner for every decision class that AI touches. Write the name down, not the job title.
  • Define which decisions require verification against a primary source before action.
  • Log the prompt, the output and the approval. If it is not logged, it did not happen.
  • Publish a one page rule on what AI output may and may not be relied upon, in plain language.
